Please be reminded that you need to provide the coach with a player registration form for the tournament if your child is selected.

Please click on the logo below to print out your child’s form.
(It is the same form that you completed to sign your child up,
you just have to print it now)




After the form has been completed, please:
  • print it out,
  • sign it, and
  • give it to your child’s tournament coach ASAP.
